Active Ageing! (Aktiv ins Alter!)

The WHO-demonstration project \"Active Ageing!\" was carried out in three districts in Vienna from 2003 to 2005. The project intended to improve the health of socially disadvantaged older people and to combat social exclusion on basis of existing resources in the community. This was made possible by a multicultural and client-centred approach, including trained social workers and psychologists visiting the target group at home to find out about their expectations and needs. People at high risk of social exclusion are isolated people, members of socially and financially disadvantaged groups such as older migrants from former Yugoslavian countries or Turkey. In general, older migrants are badly informed about asserts and services existent in the community. Therefore the project worked on two levels: firstly on the basis of the individual needs, and secondly on the systemic level, in order to improve the social infrastructure in the districts.
Community empowerment in Graz - Denggenhof

The housing scheme “Denggenhof“ in the city of Graz has approximately 1500 inhabitants. The community management project “Denggenhof“ was launched in 1999 to promote active and representative participation towards enabling all community members to meaningfully influence the decisions that affect their lives and improve their living situation. Citizens are considered as experts of their life situations and are empowered to implement innovative ideas according to their individual interests and skills.

The Graz project \"Housing for Help\", launched by the association for promoting gerontology and the studies of senior citizens at Graz University (GEFAS), opens up new ways of living together for older and younger people. Sharing the household presents an alternative form of living together to the traditional family and provides a setting in which social contact can take place.

Promoting Healthy Ageing in Rural and Semi-Urban Communities in Austria (Lebenswerte Lebenswelten)

"Lebenswerte Lebenswelten für ältere Menschen" is a best-practice model of community-based health promotion for the elderly commissioned by the Austrian Health Promotion Foundation. The three-year-project's aim is to develop, explore and evaluate innovative approaches or policies for community-development and empowerment of elderly population groups to improve their health-related quality of life and participation in social activities. The project started in March 2003 in 13 Styrian communities located in two districts. These districts differ in two criteria: population income and percentage of elderly people.

THE CRISIS INTERVENTION AND COUNSELLING TELEPHONE LINE FOR THE ELDERLY

The crisis intervention and counselling line for the elderly (800 157 157), is available free of charge to all older people throughout the Czech Republic. The line can be accessed round-the clock 365 days a year.
It is designed for older people who have found themselves in a difficult life situation due to their social circumstances or deterioration of their health condition. Availability of such service gives the clients a feeling of safety, provides them with support and guidance, thus bringing them relief and giving them hope, which makes it easier for them to cope with their situation and return to the pre-crisis level. It represents help in a crisis, including counselling and where needed, also further follow-up services for the clients and their close relatives. Currently it can be used in real time only by one calling client at a time. The goal of the project is to make it possible (in technical as well as staffing terms) for at least two clients to access the line at the same time in the “peak hours“ – when most clients use it. Another important objective is to maintain the current level, stabilize and develop the operation of the innovated line.

A Book at the centre of the attention! (Un libro al Centro)

The Province of Rimini grants to the social centres gathering older people two yearly subscriptions to newspapers or magazines at their choice and a voucher to buy a number of books. Social centres can list their requests in a questionnaire and afterwards the Provincial Government directly subscribe to magazines and newspapers and send an order directly to a newspaper kiosk. This project also promotes workshops of autobiographic writing and recovery of the individual and historical memory.

Creation of a "transport on demand" system

The project wants to extend the offer of transport for older people, through a system of "transport on demand" able to cater the needs to access health services and opportunities for leisure time. The project is thought to positively affect the quality of life of older people leaving in the most disadvantaged and isolated areas. It forecasts the delivery of services through the enhancement of the network resources and the possibilities of transportation in order to tackle isolation.

The project covered the area of the province of Modena involving the Health services and the municipalities: it dealt with the setting up of a system able to forecast the heat waves and start up a support network in order to inform, monitor and assist older people both in their houses and in care centres.
My Home is Safe! (La "mia" casa sicura)

The project carries out a shared regional planning with the aim of assessing the social sustainability of actions in favour of the current and future needs of the older people. In 2003, the "Heat Emergency" showed the need to improve the assistance to chronically ill people through an alarm system and the use of simple technology. The diffuse presence of our domiciliary care services gives us the possibility to report the danger of home accidents. The objective is to monitor our area trying to optimise the network of current services through technology (GIS, domotics…) and human resources (Caregivers, health and social workers, technicians, volunteers)

In 1/12/2004 in seven different Italian towns (Bresso-Milano; Bologna; Civitanova Marche-Macerata; Firenze; Roma, Eboli-Salerno; Palermo) this project started information help desk in the framework of already running social centres. These desks are meant for older people, their family members and their caregivers to favour the access to social services offered in the different areas. Specifically trained volunteers manage the Help desks: they have access to a user friendly ad hoc data-bank.
